Exosome Therapy After a Hair Transplant: Questions for Your Surgeon

Evidence for exosome therapy after a hair transplant is limited, and the decision belongs to your surgeon. A 2026 review found more support for PRP than for exosomes in this setting. Here is what is known and what to ask.

Hair loss can have medical causes. See a doctor or dermatologist before starting any treatment.

What is known

A 2026 review of add-on therapies in hair transplantation concluded that PRP has the most support, with randomized trials in pattern hair loss reporting gains of roughly 10 to 30 hairs per square centimetre. It also stated that evidence specific to transplantation is limited and varies. For exosomes, it described effects in lab work on cells but a lack of robust clinical validation and standard characterization (PMID 42465074).

It lists microneedling-assisted application among the emerging add-ons, with variable results and few transplant-specific data.

What is not known

  • Whether exosomes improve how many transplanted hairs survive
  • When after surgery a scalp product would be safe to use
  • Whether microneedling a healing scalp is wise

The decision is your surgeon's

After surgery, the donor and the recipient areas are healing. Complications that surgeons track include pain, swelling, bleeding, infection, folliculitis and persistent redness (PMID 41709896). We give no care instructions. Follow the written plan from your surgical team.

Questions to ask your surgeon

  • Do you recommend any add-on therapy after my procedure, and what is the evidence for it?
  • If I want to try an exosome product, when, if ever, is it safe on my scalp?
  • Would it affect how my grafts heal or what I should expect?
  • What do you use, and where does it come from?
  • Who do I call if something looks wrong?

Questions

Can I use exosomes after a hair transplant?

Ask your surgeon. They know how your scalp is healing and what they used. Evidence specific to transplants is limited.

Does PRP have better evidence than exosomes after a transplant?

A 2026 review calls PRP the most evidence-supported add-on in this setting while noting that evidence specific to transplants is limited. It says exosome approaches lack robust clinical validation.
